However, building a strong foundation begins with recognizing a few key, high-yield patterns for some of the most frequent conditions you'll encounter in clinic. This is where dermoscopy becomes invaluable, and the 'yellow dot' sign is arguably its most iconic feature. In the dermoscopy of alopecia areata, these yellow dots are not just a minor finding; they are a cornerstone. They appear as roundish, well-defined dots of varying sizes, often described as resembling tiny dandelions scattered across the scalp. What are you actually seeing? These dots represent distended follicular openings that have become filled with a mixture of keratin and sebum. They are most easily seen in fair-skinned individuals and in areas of established, non-inflammatory hair loss.
But you rarely see them in isolation. The true power of pattern recognition comes from seeing the whole picture. Alongside these yellow dots, you must actively look for other tell-tale signs. The most classic companion is the 'exclamation mark' hair. These are short, broken hairs that are narrower at the base (near the scalp) and wider at the distal end, mimicking the punctuation mark. Their presence indicates active disease where the hair shaft is fragile and breaks easily. Another crucial finding is the 'black dot,' which represents a hair that has broken off right at the skin surface, often called a cadaverized hair. When you combine the observation of multiple yellow dots with scattered exclamation mark and black dot hairs, your clinical diagnosis of alopecia areata becomes very secure. Mastering this pattern allows you to differentiate it from other common causes of patchy hair loss like trichotillomania or tinea capitis with much greater accuracy and less reliance on invasive procedures.
Skill 2: Recognizing the 'Strawberry' Pattern
Actinic keratosis (AK) is one of the most common lesions you will see, a direct consequence of cumulative sun damage. While most appear as rough, scaly patches, some variants can be pigmented, posing a greater diagnostic challenge to distinguish from early melanoma or seborrheic keratosis. This is where the 'strawberry' pattern in pigmented actinic keratosis dermoscopy becomes an essential skill. The name is a perfect visual descriptor. Imagine the surface of a strawberry: a red background dotted with numerous small, white-yellowish seeds. In dermoscopy, the "red background" is the erythematous, sun-damaged skin surrounding the hair follicles, creating a reddish pseudonetwork. The "seeds" are the openings of the follicular infundibula, which often appear as white or yellow-white, targetoid circles.
Why is this pattern so important? It helps you identify a potentially pre-malignant lesion (AK) based on its architecture, not just its scale. When examining a pigmented lesion on sun-damaged skin, finding this strawberry-like appearance is a strong indicator of a pigmented AK. However, you must look closely for additional features. Often, you will see fine, light-brown, stippled pigmentation distributed unevenly. More critically, you may observe gray dots, granules, or circles. These represent melanin within the epidermis and are a key feature that gives the lesion its pigmented appearance. Recognizing that this specific combination—a strawberry pattern on a background of solar damage with superimposed gray pigmentation—is characteristic of a pigmented AK, allows you to manage it appropriately without undue alarm. It sharpens your ability to say, "This is likely a sun spot that has become pigmented," rather than a more dangerous melanoma.
Skill 3: Analyzing Uniform Red Dots
Psoriasis plaques have a classic clinical appearance, but dermoscopy provides beautiful, pathognomonic confirmation that can help differentiate it from eczema, lichen planus, or superficial basal cell carcinoma. The key pattern to master in the dermoscopy of psoriasis is one of uniform vascular structures. Under magnification, a well-developed psoriatic plaque reveals a sea of tiny, red dots. These are dilated capillaries in the dermal papillae, seen from above. The critical adjectives here are uniform and regularly distributed. The dots are typically all roughly the same size and are evenly spread across a light red or pinkish background. They often have a dotted or globular morphology.
This vascular pattern is almost never seen alone. It is almost invariably accompanied by another hallmark: diffuse, white scaling. The scale in psoriasis is typically thick, silvery-white, and can be layered. On dermoscopy, this appears as bright white areas that may partially obscure the underlying red dots. Sometimes, you might see small, pinpoint bleeding points (Auspitz sign) where removing scale has torn these delicate capillary loops. The combination of uniformly distributed red dots on a reddish background with overlying diffuse white scale is highly characteristic of psoriasis. When you see this pattern, especially on the elbows, knees, or scalp, your diagnostic certainty increases dramatically. It’s a clear, visual signature that separates psoriasis from other red, scaly conditions which may have more irregular vessels, different scale colors (yellowish in eczema), or distinctive patterns like Wickham's striae in lichen planus.
Skill 4: Distinguishing Gray Pigment
In dermoscopy, color is meaning. While brown and black often relate to melanin, the color gray deserves your special attention and respect. Seeing gray pigment is a warning sign that should prompt a careful, systematic analysis. Its significance is highly context-dependent, and learning to distinguish its nuances is a vital skill for patient safety. As mentioned in Skill 2, gray dots, granules, or circles are a common and expected feature in pigmented actinic keratosis dermoscopy. In melanoma, gray-blue colors often appear as larger, structureless areas, dots, globules, or velvety patches (blue-white veil). This pigment is located deeper in the dermis, which scatters light to create the blue-gray hue. The key is to assess the pattern and architecture of the gray pigment, not just its presence. Ask yourself: Is the gray pigment fine and sprinkled (favouring AK), or is it coarse, clustered, and associated with other high-risk features like an atypical network, irregular streaks, or multiple colors (raising concern for melanoma)? Does the overall lesion structure look organized like a strawberry, or chaotic and asymmetric? Mastering this distinction prevents both the over-biopsy of benign pigmented AKs and the catastrophic under-diagnosis of melanoma. Always remember, gray pigment means pause and analyze thoroughly.
Skill 5: Assessing Follicular Abnormalities
The hair follicle is a dynamic structure, and its appearance under dermoscopy provides a wealth of diagnostic information, especially in scalp disorders. While we began with the yellow dots of alopecia areata, a comprehensive assessment of follicular status is a broader, essential skill. Returning to the dermoscopy of alopecia areata, a thorough examination involves a systematic search for a triad of follicular signs: the yellow dots (follicular plugs), black dots (cadaverized hairs), and exclamation mark hairs. The presence and density of these signs can even give clues about disease activity, with numerous black and exclamation mark hairs suggesting active progression.
However, your follicular analysis must extend beyond alopecia areata. In scarring alopecias like lichen planopilaris or discoid lupus, you will see perifollicular scaling and redness, loss of follicular openings (replaced by white dots representing fibrosis), and tufting of hairs. In tinea capitis, you may see comma hairs, corkscrew hairs, or broken hairs with scaling. In trichotillomania, follicles may show broken hairs of different lengths, coiled hairs, and an absence of the specific signs seen in alopecia areata. The skill lies in not just spotting an abnormality, but in interpreting the specific type of follicular change within the context of the entire clinical picture.
